Physical therapy context

Documentation sits alongside patient care.

APTA notes that many physical therapists find it difficult to document effectively while providing patient and client care. It also identifies legal, regulatory, and payer requirements for clinical documentation. [1]

APTA describes PT documentation as a legal requirement and a record that may be used to show compliance with federal, state, payer, and local rules, or by payers to assess whether services are appropriate for payment. [1]

Before treatment starts, clinics need to determine the coverage limitations and payment policies of the relevant payers. With multiple payer rules to check, these are ongoing administrative tasks around care. [1]

This is sector context from APTA, not a TendrilDesk result or a promise about any clinic’s documentation or reimbursement.

A PT-specific Medicare example

Plan-of-care certification changed for some claims in 2025.

APTA and CMS describe an exception for initial certification. The details below are reimbursement context, not a TendrilDesk outcome. [2] [3]

Before the change

Follow-up often meant repeated signature requests.

Clinics sent the plan of care to the referring provider and followed up for a signed and dated copy. PTs sometimes sent multiple requests and checked that a signature came back. Untimely signatures could leave otherwise medically necessary Medicare Part B outpatient therapy claims unpaid. [2]

For dates of service on or after Jan. 1, 2025

An initial-certification exception has specific conditions.

A physician- or NPP-signed and dated order or referral can substitute for the initial plan-of-care signature when it is in the patient’s record and identifies the patient and therapy type. The clinic must document that it sent the plan of care to the referring provider within 30 calendar days of the initial evaluation.[2] [3]

APTA describes silence after transmission as assent if the provider neither returns a signature nor indicates changes. The exception applies to initial certification; it does not exempt recertifications. [2] [3]

One clinic · one monthly service

$1,500 USD per clinic per month.

The flat price covers the owner-confirmed coordinator-led scope: HIPAA training and audit preparation, credentialing, then insurance paperwork. Payment is for service, not software access.

The intended benefit is helping clinics prepare and reduce staff administrative burden through human coordination. TendrilDesk does not promise compliance, successful audits, credentialing or payer acceptance, claim payment, fines avoided, or quantified savings.

The clinic-fit inquiry is not a service request, acceptance, or purchase. Do not send patient information, other sensitive information, or documents. Enrollment requires account sign-in and is available only when company payment setup is ready.
